HCR Home Care Names Nathaniel Sheppard as Director of Diverse Care Project

HCR Home Care has appointed Nathaniel V. Sheppard as director of its Diverse Care Project, an initiative aimed at reducing health disparities among Black residents age 65 and older.

In his new role, Sheppard will oversee efforts to address unmet health care needs by identifying and eliminating barriers to accessing home health services. The project, launched as a pilot during the COVID-19 pandemic in partnership with HCR Cares, is now being expanded under Sheppard’s leadership.
Sheppard brings more than 30 years of senior-level experience in program management, operations and facility oversight. He most recently served as director of operations at the Anthony L. Jordan Health Center. His previous roles include director of housing, energy, emergency food and employment programs at Southern Maryland Tri-County Community Action Committee Inc. in Hughesville, Maryland, and director of energy conservation programs and facilities coordinator at Action for a Better Community.
Sheppard holds a Doctor of Education in executive leadership from St. John Fisher University and a Master of Business Administration from Medaille University. He lives in Hilton, New York.
The Diverse Care Project is funded in part by a grant from Monroe County. Additional funding for HCR Cares, HCR’s nonprofit partner, was provided by the Greater Rochester Health Foundation and The Max and Marian Farash Charitable Foundation.
